Since insurance plans cover 100% of treatment, we ask that you pay your deductible and your portion of charges at the time services are rendered.

The plan requires an annual membership fee starting at $400 for an individual plan. You will also receive a 15% discount on general and major dental procedures. Also, children receive free fluoride treatments every six months as needed.
